Northeast Region Reporting Guidelines

The following reporting guidelines will be utilized for communicating information regarding reportable incidents. All oral and written communication should be timely and accurate. To ensure everyone is appropriately notified, a brief written synopsis containing important and relevant details is expected no later than the end of the shift in which the incident occurred.

The synopsis should be sent via GroupWise, from the Warden or Acting Warden, to the Regional Director, Sr. Deputy Regional Director, Regional Executive Assistant, Regional Counsel and the Regional Correctional Services Administrator. If a tele-message is required, please limit it to one message, if possible.

Serious Assaults to Staff or Inmates Requiring Medical Attention

Staff Death

Inmate Death (Not Expected)

Inmate Homicide or Suicide (Including Serious Attempts)

Sexual Assaults (Staff or Inmates)

Arrest or Detaining of Visitors

Civilian Demonstrations or Protests

Discharge of a Firearm

Use of MK-4 or MK 9 OC Dispensers

Use of any Less-than-Lethal Munitions

Inmate Hunger Strike (Involuntary Feeding)

Outside Hospital Trips for High Profile/Max Custody (Other than Routine)

Significant Issues related to High Profile Inmates

Escapes or Serious Attempts from any Institution

Any Type of Inmate Disturbance (Organized or Spontaneous)

Any Serious Fires Resulting in Injury or Significant Property Damage

Initial Application and use of any Ambulatory or 4-Point Restraints (Soft or Hard)

** Other notifications that do not necessarily require a 2-hour limit:

Covert Operations (Require Prior Approval of OEO, RD, and AD CPD)

Tragic Incidents involving staff and/or their immediate family
