Epstein received an incident report 7/23/2019, for code 228, Self Mutilation. It was expunged based on insufficient evidence, by the Disciplinary Hearing Officer (DHO) on 8/1/19, and signed by the DHO 8/2/19.

Also, following up on our conversation, we do have his Central File. There is limited information in there due to his being a pretrial inmate. There is nothing there concerning his need to be housed with a cell mate. In addition, we were able to recreate the 292 but that did not reflect anything on the need for a cell mate either. We continue to look for evidence that the unit officer had (other than the verbal instructions which we have noted).
