Although having a religious preference of “UNKNOWN,” inmate (86461-054 - 9N) came on the Jumah move and made an ALL EYES ON ME grand entrance into 5N, shouting and waving his arms and hands as he walked to the Chapel, trash-talking with the inmates secured on multiple 5N tiers, getting many inmates worked-up & causing a lot of noise in the unit. I told him his behavior was unacceptable and that I expected him to leave in a calm & quiet manner when the service was over. Then, in the middle of the Muslim sermon, inmate (86521-054 - 5N) came up to the secure Chapel door and proceeded to engage in a non-verbal conversation through the door window with while he was seated in a chair for the Muslim service, and was more than willing to engage in the charade-based conversation. As I approached the door, inmate went to an unknown tier/cell. I then stood near the door and a few minutes later, inmate came half-way up the stairs to the Chapel trying to communicate with again. I addressed the matter with them both. respectfully apologized but just ran his legendary mouth, hence his 30 day restriction from chapel services.

The RAC Ms. notified me on Thursday, July 18 that the Saturday volunteers (Edgar and Blanca Vinces) were not able to come in until they completed their training on July 29. I notified them via telephone so they would not make the trip to the institution. Edgar confirmed that he plans on attending the training on July 29, and his wife is trying to make arrangements to get off work to do the same. Also, I delivered memos to each housing unit and had Control announce the cancellation of the Saturday service.

After I delivered the sign-up sheets to all the units for the English Christian service on Sunday and after the 10am count cleared, 5N Officer notified me that he received a drop note about a gang fight planned during the church service that day (see attached). He notified the Lts., and the Lts. notified me that they felt it best to cancel the service. I immediately notified volunteer Daniel Washington, who confirmed he would be able to come on his next scheduled Sunday to provide the Christian service on Aug. 4. I also had control announce the cancellation of the Sundav service.

I added inmate Epstein to the CFSA roster, and interviewed inmate Encarnacion, adding him to the roster as well.

I escorted the Rabbi on Sunday to all units with active Jewish inmates, including 11S which was locked down.

Synopsis: On July 23, 2019, at 1:27 a.m., staff responded to a call for assistance in the Special Housing Unit (SHU). Upon arrival, the Operations Lieutenant found inmate Epstein, Jeffrey, Register No. 76318-054, lying on the floor in a fetal position. The Lieutenant called out to the inmate; however, he did not respond and she noticed red marks on his neck. The inmate was placed in hand restraints and escorted to the Suicide Cell and placed on Suicide Watch. While on watch, the inmate indicated that his cell mate had attempted to kill him and had been harassing him and he had informed his Attorney of this matter.¶
Medical Assessment: The inmate was medically assessed at 6:30 a.m., by the Physician Assistant and his findings were as follows:¶
He is ambulatory, oriented x 3. In no apparent distress, smiling during this clinical encounter. Alleges, that he does not know what happened. Cannot explain the marks on his neck. Responded: “I don’t know”.¶
He does not want to talk of the events leading to the marks on his neck.¶
He does not look in any distress or pain.¶
Has a circular line of erythema at the base of the neck. Reaching 2/3 of the neck circumference, 2 inches wide, sparing the back of the neck. Has one section of this erythema in the front with marks of friction.¶
No inflammation, no deformities, no hematomas, no lacerations, no tenderness. Patient moving his neck without any restriction. Denies having any pain or discomfort. Denies¶

any respiratory problem.¶
Has another small erythema on left knee about 2cm in diameter (mild).¶
Psychologist Assessment: He was evaluated on today at 9:10 a.m., and it has been determined he will remain on Suicide Watch for further observation.¶
Current Status: At this time, we are reviewing his case for placement in the general population in Unit 7 South. We are currently looking for the appropriate cell mate.¶
