This investigation is being conducted, pursuant to the Inspector General Act of 1978, as amended. This investigation pertains to job performance failure and security failure. MR. : If Mr. , being that you were, I guess, just, you know, I know it was Acting, but he was the Acting Supervisor on August 19th, if he knew that the cameras were not recording in the facility, do you think it’s acceptable that he left the facility, knowing that they weren’t working?¶
MR. : No. I wouldn’t think that would be acceptable.¶
MR. : If he informed you that the cameras were, in fact, not working or recording on August 9th, would you have told him to fix it?¶
MR. : Yeah. So, if I was informed that they weren’t working, I would say, get it fixed. Maybe call around for suggestions on how to fix it or get a contractor that can fix it. I mean, it’s pretty important.¶

Maybe some lieutenants, a captain, to see if cameras are working. You know what I mean? So, like, they can view cameras at any time. But as far as what, like, servicing them or checking them out regularly, it would probably be , maybe Ling, if he did, at all. And maybe the Facilities Manager,¶

MR. : Okay. So, and not specifically, can you just explain the process? What would happen? So, if the cameras go down, what happens?¶
MR. : So, obviously, somebody would have to notice, somebody that has access to be able to see specifically that the camera wasn’t working, so it would be SIS lieutenants, maybe, watching the cameras. People with camera access, and then from there, you would submit a work order. Nine times out of ten, that would be done through, like, the captain’s secretary or something like that. Maybe a couple lieutenants would have access to be able to submit a work order. And then usually, the Facilities Manager would get the work orders and they would, he would hand them to the workers and say, this needs to be fixed. You know, and especially if it’s something as important as a camera, I would think that it¶

MR. : Okay. All right. So, what actions should have been taken on or around August 9, 2019, when recognized that the cameras were not working properly? So if he went in there around 1:30, said, yep, these things are all offline.¶
MR. : Yeah.¶
MR. : What should have he done?¶
MR. : I mean, I’m not a com tech, but I would assume you would try to fix the problem, depending on what the problem was. You know, and if you couldn’t get it fixed, I would notify management, notify the Operations or SHU lieutenant, make sure nobody was placed in that cell, specifically, or whatever area the cameras weren’t working in, for safety. And then work on getting that fixed, you know, from there, whether you have to get a contractor or order parts or whatever, but yeah, I would make sure the area or cell, in this case, wasn’t being used, you know?¶
MR. : Being that you were Acting for Mr. , should have he notified you?¶

MR. : Do you have any knowledge of who should have gotten him a new cellmate?¶
MR. : It depends, if he was designated to have a cellmate or not. Then the SHU staff would have had to realize that, you know, when that guy, that inmate, left for court, you would say, oh, he’s, you know, designated, you know, don’t cell alone, basically. And you would have placed another inmate in there, but I’m not sure what his exact designation was.¶
MR. : Okay. What are some of the systematic problems inside the MCC, and specifically, the SHU that allowed for Epstein to die?¶
MR. : I guess, staff not doing their job. But I would, I mean, I would just like to say, personally, as somebody that has worked SHU many times, I mean, it’s crazy that people didn’t do their job, but I think personally, that if he wanted to kill himself, you know, if you’re only required to do rounds every 30 to 40 minutes, he could have killed himself after rounds, too.¶
