This investigation is being conducted pursuant to the Inspector General Act of 1978 as amended. This investigation pertains to job performance failure and security failure. I’m writing that in here.¶

Anything communication-wise. If anybody’s having a problem, they usually get to me. Even something as simple as a voice mail. They’ll say he my machine isn’t working and I go take a look at it. And where somebody’s computer is not working. So.¶
MR. : And since you mentioned also daily when you come on the job at 6:00 a.m. you check to see if the phone, cameras, everything is working?¶
MR. : I don’t have to check.¶
It’s - there’s no set standard or like okay you should come in and check it. You just like kind of look to see what’s going on. You kind of know what hasn’t been working and what’s been working. You kind of make a checklist for yourself. There’s no set standard of what you should do at that time. Now there is more stuff that you do. Day to day. Where before there was really no standard of okay you come in, you start, and this is the first thing you’ve got to go and do. And no. There was never anything written down as far as what should be done during your shift.¶

MR. : Okay. And once it was recorded, do you remember checking the cameras that day? Seeing if it was working?¶
MR. : I checked them later in the day. I went up to the video room. Because we were having - the thing about this whole process was when I came here, the first thing I saw was the video room. And the biggest thing was all the equipment in that room was out of date. It was old. It was - I came from another institution. Like I said before where everything kind of was modern but it was more up to date than this. And so like the camera system I had was maybe five to ten years old. And when I came here, this one was over ten years old and it was 365 a day. Like 24 hours for the like the last 12 years. The system was always on. And something as simple as shutting down the time and change - because you got to get recording times right. When the seasons change, you would have to wait like a whole day to shut down a recorder. It takes a whole day or it to come back up after shutting it down. And where I came from, that was not the standard. Like it should be like an hour maybe¶

thirty minutes. Maybe even ten minutes if it’s a newer system. You shouldn’t even have that much down time as far as it recording. So one of the first things I did when I came here was I addressed that - the recorders being upgraded. And -.¶
MR. : Who did you address that with?¶
MR. : Um my Facility Manager. I talked to upper management.¶
MR. : Who?¶
MR. : But like during this whole time I was doing -. I actually started the camera project and that’s why I was doing overtime around this time. And we had people come from other institutions. There was a camera project to upgrade the cameras.¶
Everybody’s fear was the actual cameras not the recording software of the cameras. But I stated over and over again the problem you’re going to have is the recorders. And we even had the new recorders. They were in the warehouse. We have them stockpiled. They was ordered. But we were on the bottom of the list as far as install. So even if we had the new¶

digital system. The old system would never show me like the recorders failed. Like if there was an error on the old software. Like I would have to physically be in the room to say hey. Because the room was upstairs on the third floor. Which I didn’t have access for. You would have to go in the room and then you go see like - because there’s a redundant drive. So you have like an A side and a B side. And say if the A side isn’t working, you have one drive goes back. That’s not a problem. As soon as the second drive goes bad, that’s when you run into problems. And after the second drive runs into problems, that’s when you stop recording. And once that happens, you have to take the drive out, and we have to scrounge around for old drives and see if those will work. During that time I actually - prior to that I actually purchased maybe 24 drives to actually redo a whole - like A or the B side. I was actually going to do that.¶
MR. : Did you know at that point that SHU cameras weren’t recording?¶
MR. : At that point towards the¶

It kind of worked the same, as far as the software side goes, as far like as like the hardware, it was different. So I would always have to call them for like tech support. Like okay I’m having a problem today. Can you document it? You know it builds our case when I have to submit stuff to Regional Office. Needless to saying that doesn’t work. That’s not going to fly. And usually they help me troubleshoot through the whole process of what I should have to do - A, B, C, okay take this drive out, put that drive in. And like previously, probably six months before that, we actually had a mother board - which was my main¶

MR. : How come some recorded on -?¶
MR. : Okay. Oh there’s going to be another problem too. Okay. So there’s an A side and a B side. And it’s supposed to be redundant but it’s not redundant because it’s not really redundant when one side goes bad so it just knows to record channels. Let’s say one through fifty is on this side. And 51 through 100 is on this side. Everything is working. It doesn’t work. Okay. I can take over for this one because it’s not working.¶
But as soon as that one goes, I can kind of record over here. As soon as the second drive goes, I have to just record what I’m recording. I can’t no longer help out the other drives.¶
MR. : Okay. So some channels inside the SHU were recording on one drive?¶
MR. : Yes.¶
MR. : Some were on the other drive?¶
MR. : Yes.¶
MR. : So the ones - which drive was the one that stopped recording altogether?¶
MR. : I believe it was the first¶

MR. □□□: If someone had to come in and out of the SHU and control had to bus them, was there a way control buses was —¶

MR. □□□: The way that the doors work at that time was - because we changed some of the cameras. But before control pops the door, they pretty much acknowledge that -.¶
Well before - especially with 9 South. 9 South is different because the officer has the key on their hip to - there’s an inner door inside here. And they won’t even - well they’re not supposed to - crack the outer door without somebody in SHU calling for that door. So I can’t go up to 10 South - or 9 South - right now and say hey, let me get the 27 door.¶
Somebody from SHU has to call for the 27 door. Because the inner door they have the key before you come inside of 9 South.¶
MR. □□□: But there was no cameras at that point?¶
MR. □□□: No. There was a live¶

keypad. Not a lot of people have access to that. So like SIS would have it - at the time.¶
Um control center would have it. And um lieutenant’s office don’t have it. So it’s SIS, SIA, I have access, and that’s pretty much about it. And then our command center.¶
MR. : Is there something that they would have to key in to see? Or is it just running?¶
MR. : No. You would have to key in the cameras to see the actual camera call up. You have to call up for them hit the button and know what camera it is to call up.¶
MR. : Did anyone like the SIS office or even the control have like the cameras - you know like the feed just running on their screen at all times?¶
MR. : Yeah. But they would have to go through all the cameras. They usually just - it just sits on one camera. It doesn’t keep scrolling through all the cameras.¶
MR. : Okay. Do you have anything on that?¶
A lot. Yes. A lot.¶

And did you know all of them were not recording at that time? Aside from the one?¶
MR. I didn’t know exactly wasn’t what wasn’t recording on the recorder.¶
I don’t know exactly. Which ones weren’t working. I just knew that recorder was down.¶
And how did you know that?¶
MR. : As soon as I went in I saw the drives. Because the drives usually they’re green on everything that’s working and amber like there’s an emergency or something’s wrong. And I saw that there was a few ambers. So like it wasn’t recording. Then I called at the company. And they said I have to start rebuilding. And what start rebuilding means is I have to pull that hard drive out and start putting fresh hard drives in. and then the system knows like it put a new hard drive in it starts to sync up and does this thing where it starts to go through. It does it all of its checks and starts recording. And I would pretty much take a day.¶

MR. : Okay. The system is very old. But that day on August 9th when all the SHU cameras went down, that had nothing to do with you? It was just coincidental that they went down. MR. : As far as the recording, yes.¶
MR. : Okay. So when you specified as far as the recording, can you tell me why you said that? Is there anything else? MR. : Because people believe that when cameras don’t work, like the camera’s not working. The camera could be throwing out video because the live feed - like I said - goes to the control center. They see the live video when they’re popping the door. So there’s live video going - getting fed. It’s just the digital side of it. The recording side is not working.¶
MR. : Okay. Was there a camera towards the back room?¶

sometimes when the software isn’t right. So it bogs down the system. And we’ve had problems where the recording stops, but we have a redundant recorder but not all the cameras would go down per se.¶

Now were there any cameras, recordings, or hard drives or anything that were sent to try to have the recordings recovered?¶
MR. : Uh well the whole thing with everything not recording that day was the way the system works. And we want to woke the vendor and FBI because they had to talk to - the way the system works is if the two drives are bad it’s not going to start recording video. It’s not going to go on a different drive or you’re going to see bits and pieces of video from that day. You might see old video. But as far as new video from that day whenever it went back you’re not going to see any video.¶
So we had received information saying that video was attempted to be recovered by the FBI.¶

MR. : Not necessarily. It could be garbage. Garbage can record anything that’s a good thing what is not a good thing. This (Indiscernible *00:48:51) was recording. That’s why I say that when a camera is and or good, like I can have a camera that just looking at this and you’re saying okay it’s good but it’s not looking at the whole room. This is nothing. This is garbage. It doesn’t. The camera just knows it’s working until I fix it and then it records the whole room. Then you know it’s working.¶
MR. : When you went in on the 10th to take a look at the video, did you actually see the recording files? Or did you not see any files at all?¶
MR. : I just know the drives were bad. And I think I tried to -. Yeah¶

access. SIS had access. I had access. And I believe computer services. And maybe a few staff members here and there. But they just looking at live video - nothing recorded or (Indiscernible *00:50:08) video.¶
MR. : SO that was my next question. Could they have done anything when they logged in to manipulate the -?¶
MR. : No. You can’t stop the -. The newer system you can. The old system you pretty much have to go in there and manually do it. Like you can’t just -. You would have to go on the recorders and -. Yeah. I don’t think anybody here knew - knows how to do that.¶
MR. : Did anyone outside the institution have access? Like because it was on the network, does like someone at the central office or anyone have access to or no?¶
MR. : From what they told me there’s no back doors to it. So.¶
MR. : So it was a close network.¶
MR. : Closed. Yeah. Closed.¶

MR. : He was kind of like somebody was just holding him. So I can’t really tell if he was going this way or that. But somebody was behind him physically holding him like this trying to move him.¶
MR. : Were they trying to get him out of the corner and bring him out?¶
MR. : Yeah. I believe that’s what was going on. But there was no way to put him there because you can’t put him on the ground to do anything. So that’s when we free to get the stretcher and AD machine.¶
MR. : Why couldn’t they put him down on the ground?¶
MR. : I usually put them down on the ground. But usually it just second nature. You always get a stretcher. Because you don’t want none of the inmates seeing you work on an inmate. It never looks good. So you kind of - you just - everything is just so fast. Like it just moves just like a - it’s not like you wait or hesitate. But you just kind of know what to do. There’s no hesitation. But you just know what’s going to happen and you never really want to work in front of the inmates or the¶

No? All right. And then going way back to our initial conversation you said that you put in a work order for Was that on the 9th?¶
MR. Um probably before yeah. Of July. Um.¶
no. that would have been August 9th. The day before Epstein was found.¶
MR. Yeah. I was just troubleshooting with them. I was on the phone with them. It’s not really a work order. Just you know I call them up. Hey, this what’s going on what should we do.¶

MR. : Uh maybe a year before or six months before. It was just a constant thing as far as the cameras not working.¶
Because we was trying to get upgraded. We were trying to get money spent. So we have to create - any time there’s a problem, I have to create a work order or tell like the regional office like there’s a problem. Because it’s a work order over 10,000 so they have to put money aside to do the work. Like it’s not a quick fix to fix the recorders.¶
MR. : And these recorders were maintained in house or only by ?¶
MR. : The work orders we have are maintained here in house.¶

MR. : I already knew we were having issues. And I think I got the hard drives out. Because I was going to take them up there and see what was going on.¶

MR. : Um I think it’s switched.¶
It could have been recorder one and then recorder two. I don’t think it was just the same recorder all the time because one recorder was actually the whole motherboard have to get replaced. And that was again I think that was recorder two. And before drives would go out on one or two, so it was just random. They were old. They just needed to get replaced.¶
MR. : And at that point you knew that the drives were not recording.¶
MR. : Yes.¶

We’re thing the curve. We got IP cameras pretty much everywhere. But I’m just saying overall I just with the Bureau was more proactive. And like when I came to this institution for instance like there was only one camera in a housing unit. When I was in Brooklyn, I was custody, it was cameras all over the place. I went to Indiana and I was a contact at a complex which was a high, medium, low, there’s a camp, there’s a death row.¶
There’s cameras everywhere. I come to this jail, New York City. And I’m expecting cameras everywhere. And one of the first things was like oh we don’t have money for that. we don’t have money for this. We don’t have money for that. and I just kept saying hey, you guys got to do something. This is a problem. God forbid this is what happened. But I just wish there was more proactivity. Like -.¶

MR. : And training too. Like just getting new equipment. That’s not it. Also train the staff. Because even like when I learned how to do the cameras stuff, it was just me over the phone with . I never really went to a physical training. Hey, this is the system you have. Take this drive out, do X, Y, and Z. I’m just like I’m on the phone. I’m looking at it. Okay. The light is this color. This is - I’m a smart guy. I could figure it out. But at the same time like -.¶
