EFTA00131274 8/30/19 USAO SDNY Interview ## residence 2004 - started for BOD - → other MCC- 1 year ago GS - current SHU + ops - both, was on SR-Z, activities +ops logging movement of building + overtime issues - responsibilities for operations ,does rounds in SHU, goes down range + addresses any issues w/ SHU inmates - signature sheet for Ops Lt. ,Marning watch Lt. puts final signature + then routes to captain → signs + put in logbook - no involvement during cant for SHU during week unless emergency cant - nauld check to make sure rands EFTA00131275 6 A-2 pm - as SHO Lt. no PM SHO Lt. @ MCC, other institutions have it BOP annual refresher training last one @ 2ce Fed. SHO training - quarterly w/ new staff members (officers) assigned to stk anyone mandated—in post orders - suicide prevention training is part of it, goes over signs what to do if happens, etc. - If inmate on suicide watch, inmate will have cellmate - As ops it, migvisits STU approx. once a day - learned of Epstein once in MCC, heard about first suicide attempt but wasn't working then he was on suicide watch then brought back upstairs, cellmate was placed with him wasn't at any SW meetings Epstein EFTA00131276 placed Epstein w/ -Cos during shift knew to keep them together (day watch) them together (day watch) told Epstein he would be placed w/ him - no issues not list - psychology - inmates who had been on suicide watch/ require a bunky (cellmate) - OT at STU - requires being mobile + getting paperwork ready for AM -recalled email, sent before was placed w/ him - 8/9- ops Lt. 8A-4pm (scheduled) actually wanced 6 A-2pm moved in morning new cellmate wasn't placed was waiting to see who new cellmate would be decided Captain or Psychology would make that decision Anyone Epstein couldn't have been placed w/ just anyone -didn't tell relief or captain that had left EFTA00131277 didn't speak to any of the co's that were coming on to next shift didn't mark 8/10 . heard about it on news - Cos get from 9+n -10+n floor via stairs, can take elevator from 9+n -11+n 10+n -is not a full floor, like an attic - bathrooms on 9+n + 10+n for Co's females mostly use 10+n old more private/comfortable - If do rand you pick up phone, and control cets you in / same as bathroom
