Good morning, Darren!

Just fill in all of the blanks on 2 of the forms, describing each of the aircraft (make, model, serial number, registration number), this time showing your new company name in the applicant block. Be sure that a physical address is shown. Also, be sure to print your name, in addition to signing the form. Your title "Vice President" should also be shown in the appropriate block. You can keep the pink copies, and send me the white & green pages of each form.

Then, just mail or overnight the forms to me, here at AIC.
