Brief medical history: 66 year old white male with health problems noted as hypertension, hyperlipidemia, sleep apnea, pre-diabetes, and constipation. Intake on 7/8/19. Was found unresponsive on 8/10/19 with ligature marks around neck and posterior mottling.

Pertinent objective data: 7/30/19: Seen by physician for numbness in right arm, sleep apnea. Noted to previously have been on Toprol XL - Not ordered; however, all blood pressure and pulse readings in flow sheets within normal limits. Pertinent labs: 7/9/19: HbA1c: 6.3%, Total Cholesterol: 216, Triglycerides: 413, HDL: 31, Glucose: 102

Had a cell mate, and interacted with him. Noted that he did not appear to be in distress and SHU staff did not report any unusual behaviors. PDS encounters daily from 7/23/19 through 8/1/19 Prior to incident below, PDS encounters noted on 7/8/19(intake), 7/9/19, 7/10/19, 7/11/19, 7/16/19, and 7/18/19 History of past suicide attempts and description: Placed on suicide watch on 7/23/19. Noted to have an incident with string/rope around neck causing damage. Denies self-harm or knowledge of the incident. Was drinking water and then woke up snoring. History of drug abuse: Denied
