A review of the 292's at MCC New York was completed for the week of 8/4/2019, with the following discrepanci es noted:

- I) Missing OIC signatures;
- 2) The same staff member signing as OIC for 5 consecutive shifts;
- 3) Missing Health Services signatures;
- 4) Recreation not documented;
- 5) Meals not documented.

Please take the appropriate corrective actions. Thank You
